Medicare Patients May Be Surveyed About the Care You Provide

From July to November, the Centers for Medicare and Medicaid Services (CMS) mails the Health Outcomes Survey (HOS) to some members of VNS Health Medicare plans.    

The HOS asks about the member’s mental and physical health, physical activity, incontinence, and falls risk and prevention. The survey is used by CMS to assess health plan performance in the Star Ratings Program. Please encourage your patients to complete the survey if they receive it. Members’ answers on these surveys affect our quality scores and reputation, which can drive enrollment and lead to more patients for you.
