Help Us Enhance Patient Access

While we have a strong presence in New York City and the surrounding area, we are actively growing our membership across New York State. VNS Health Medicare is now available in Albany, Rensselaer, and Schenectady counties, and as of May 1, 2024 our MLTC service area expanded to include Genesee, Niagara, Orleans, and Wyoming counties.

As part of this effort, we need your help making sure your organization’s information with us—such as the locations and areas you serve—is complete so patients and prospective patients can easily find you.  

We ask that you: 

• Log into our provider portal and check that the information we have is correct by clicking the attest accuracy button under the “My Account” section. If it is not correct, submit changes with the request change button.  
• Use this form to notify us about any additional locations or counties that you serve.  

Please validate your information every 90 days.
