We reward you, so you can reward yourself!
As a member of VNS Health EasyCare (HMO), you’re automatically enrolled in our Member Rewards program. You can earn rewards for completing certain health activities, like seeing your doctor or getting recommended screenings. Earn up to $30 for each health activity you complete.
You can earn rewards by completing the qualifying activities listed below.

How Will I Get My Rewards?
You must be an active member of the plan to receive rewards. We’ll track your progress using claims your doctor submits to verify that you completed the health activity. The first time you complete an eligible activity during a given year, we’ll send you a reward certificate showing the total dollar amount you earned for completing qualified health activities. Then during the remainder of the year, you’ll receive a new reward certificate for each quarter (3 months) in which you complete at least one eligible activity. A redeem by mail form will also be included so you can choose your gift card(s) or redeem online. See the schedule below for when you might expect to receive your rewards.
The gift card cannot be used for gambling or to purchase alcohol, firearms, or tobacco, and it cannot be converted to cash.

- Before you leave your doctor’s office, make sure you schedule your next visit.

You will receive a reward certificate in the mail showing the award value or total dollar amount you earned for completing qualified health activities. Call us, if you have any questions or need to update your address.
Fill out the mail-in redemption form at the bottom of your reward certificate or follow the instructions noted at the top of the certificate to redeem online. Note: you can split your rewards in up to (2) different retailer gift cards.
Rewards will be awarded during specific timeframes. See schedule outline of when rewards will be issued, above.
Physical gift card(s) will be shipped within two to four weeks after selections are made. In most cases, if selected, digital gift cards will be available for use right away.

Yes. Gift cards will expire within thirteen months from the date card is issued. Be sure to use your card(s) before then.
