How to Obtain Authorizations on the Portal

Medicare Prior Authorization Requirements

You may call Provider Services at 1-866-783-0222, weekdays, 8 am-8 pm with any questions.

For Claim Disputes: All Claim Disputes must be submitted through our Provider Claim Dispute Form. Examples of when to use the Claim Dispute Form include:

• Coding denials
• Underpaid/overpaid claims
• Invalid procedure code/revenue code/diagnosis code
• Incorrect modifier
• Denied for authorization and provider has authorization letter

For Claim Appeals: If your claim is denied and you wish to challenge the decision, you can use the grievances and appeals This will lead to an internal clinical or administrative review of the denial. Examples of appealable denials include:

• Services not authorized
• Not medically necessary
• Noncovered service
• Noncovered benefit
• Benefit exhausted
• Charges previously considered

  The Claim Appeal must be submitted in writing, via fax or mail.

If you have not received your Healthy Extras or OTC Card, please call your care team at 1-866-783-1444 (TTY: 711) 7 days a week, 8 am-8 pm (Oct. – Mar.) Weekdays, 8 am-8 pm (Mar. – October.). 

If you need help activating your card, please follow the instructions located on the sticker on the front of your card or call the phone number located on the back of your card.
